REGISTRATION FEE: (check made payable to Fairport Pop Warner)

•  $125 Per player, $325 max per family
•  $10 Deduction per additional player

"MANDATORY WEIGHT LEVELS" - FOOTBALL
- Please indicate level of play based on the appropriate age/weight of your child (see below).
- Fairport Pop Warner reserves the right to change player levels based on inaccurate information given at registration.  Movement will be made only after parent contact.
- Please weigh your child when completing registration forms. 
- It is the responsibility of parent/players to monitor their weight until equipment handout in June or July when they will be weighed again.  If the player is 6 pounds or more over the maximum weight level, he/she will be asked to move up to the netxt level (as long as age is applicable).  National Pop Warner rules do not allow players that are 6 pounds over the maximum weight to participate in practice.
- Certification takes place at the end of August.  At that time, players within the age/weight mandated levels and approved paperwrok, will be considered "certified" to participate in the first game.  Parent/coaches of those payers not "certified" will be advised of future certification opportunities.

2011/2012 FINAL Report Card

• National Pop Warner Medical Release form signed by physician along with signed parent questionnaire.  Physician signature must be dated after 1/1/12.
